Mission Statement:
The Sports Medicine team at Dakota State University is committed to providing comprehensive, high-quality medical services to every student-athlete in a very timely fashion. On-site medical services provided include, but are not limited to, prevention, evaluation, and treatment of injuries and illnesses, rehabilitation services, and pre-participation exams. The primary sports medicine team is comprised of three (3) state licensed Athletic Trainers and a team Sports Medicine physician. This professional staff is responsible for the health care and safety of over 400 student-athletes.
Student Athlete Medical Paperwork Checklist

ALL MEDICAL PAPERWORK/FORMS ARE TO BE SUBMITTED ELECTRONICALLY THROUGH THE ATS SYSTEM
New & Transfer Student Athletes:
- Create ATS Profile – instructions for creating your profile will be sent out to athletes by their coaches; forms under the e-files tab must also be filled out to complete the ATS profile
- Physical Form – must be completed within 6 months of arrival to DSU; use form provided and upload ALL 4 pages to your ATS profile under the e-files tab; must be done by an MD, DO, PA, NP (chiropractors are not accepted)
- Sickle Cell Waiver – upload signed form to ATS profile under the e-files tab (waiver is all that is required, sickle cell test not required if deemed unnecessary by physician)
- Insurance Requirements Acknowledgment – upload signed form to ATS profile under the e-files tab
- Photocopy of front & back of insurance card (included in creating ATS profile)
- ImPACT baseline concussion test – instructions sent out to athletes by the coaches, confirmation PDF to be uploaded to ATS profile under e-files tab
Returning Student Athletes
- Returner Medical Information Page – upload to your ATS profile
- E-Files: 6 forms (excluding med history & physical) – forms can be e-signed under the “Forms” tab
- Updated copied of the front & back of your insurance card (if changed from previous year)
- Returning Health Information Page
