2026-27 Student-Athlete Information Check List

Here is a check list of information that will need to completed before participation.

New Student Athlete Check List:

1. Medical Paperwork (ALL medical paperwork must be completed and turned in to the athletic training staff prior to participation) – sports medicine information letter and concussion management policy found on the sports medicine page
   • Create ATS profile (instructions on how to set up your profile will be sent to athletes by the coaches)
   • Physical Form use the provided form; must be completed within 6 months of arrival to campus; must be done by an MD, DO, PA, NP (chiropractors are not accepted)d; upload all 4 pages to your ATS profile
   • Upload signed copies of the following forms (Insurance Requirements Acknowledgment & Sickle Cell Waiver) to your ATS profile
   • Provide a photocopy of the front & back of your insurance card (uploaded to your ATS profile)
   • Complete an ImPACT baseline concussion test – instructions sent out to athletes by the coaches
   • ALL MEDICAL PAPERWORK & FORMS ARE TO BE SUBMITTED ELECTRONICALLY THROUGH YOUR ATS PROFILE

3. Sports Information Questionnaire
4. Make sure you have a real ID (one with the star) or passport for air travel with teams
